PCMH is a team based approach to providing total health care to our patients. The team of health care professionals is led by the physician and includes nurse practitioners, nurses, medical assistants and support staff with the goal to provide continuous and coordinated care.
As an active member of this team, the patient will have the opportunity to explain what is really important to them. Together, the patient and their team can work on a personalized care plan and set goals for a healthy future.

Kudos to Harbor Medical Cardiologist Michael J. Hession, MD, who has been honored by his peers of the Norfolk South District Medical Society as the district's 2015 Community Clinician of the Year.
The Community Clinician of the Year Award was established in 1998 by the Massachusetts Medical Society to recognize a physician from each of the Society's 20 district medical societies who has made significant contributions to his or her patients and the community. The Norfork South District has nearly 500 physicians who live and work in the South Shore communities.
Stanford University's Clinical Excellence Reserach Center (CERC) team recently visited Harbor Medical as we were identified as delivering high quality endocrinolgy care affordably. The Stanford team met with key Harbor Medical physicians and staff to review the model of care, clinical practice and quality performance. CERC hopes to contribute to reducing the rate of health care spending and improve the overall quality of health care.
